We have heard from several cruisers recently that checking into or out of Belize is very inconsistent. Some boats have been charged nothing, some several hundred dollars (US). One port captain reportedly is charging US $5 per person per day for some sort of cruising permit. Does anyone have recent (like in the last couple of months max) direct experience with cruising in Belize?

I don't have any experience with Belize, so I went to Noonsite to see what was reported there. Noonsite, Belize

Sounds a bit like most Latin American countries. Fees vary depending on the official sometimes, as mentioned in Noonsite. It could be every contradictory report you've gotten was correct - for them. Sometimes some ports are more difficult than others, and that information is usually part of the information on the various cruising nets in the area, so it's hard to anticipate.
